Creating Lasting Change

The goals of our work and the work of our grantees are improved community health and health equity. There strategies for achieving these goals vary depending on the community, barriers, opportunities and partner capacity. One powerful strategy is focusing on solutions that provide lasting change.

We work with partners to change structures, policies or processes, as well as shift resources, values, power and mindsets to advance health equity. Changing and shifting these aspects of our communities can provide greater health equity today — and build a sturdy framework for future health equity.

Achieving health equity is a perpetual goal and process that persists beyond grant timelines. With the help of our partners, we want to reduce barriers and improve community health across Minnesota now, and for generations to come.
